New Study: Tirzepatide Outperformed TRT for Low T

I found a recent study about tirzepatide, I think every guy should know about.

Italian researchers published a pilot study [1] in Reproductive Biology and Endocrinology looking at 83 obese men with low testosterone.

All of them had what’s called “metabolic hypogonadism”.

Their testosterone was low because obesity and insulin resistance were suppressing their hormonal system from the outside.

Traditionally, these men get put on testosterone replacement therapy.

Makes sense on the surface.

But TRT doesn’t fix the underlying obesity or insulin resistance that caused the problem in the first place.

It just replaces the missing hormone and calls it a day.

The researchers wanted to ask a more interesting question: What if, instead of replacing the hormone, you fixed what was suppressing it in the first place?

What the Study Found

They split 83 men into three groups. All followed the same low-calorie diet and 20 minutes of daily walking.

  • Group A: Tirzepatide (starting at 2.5mg/week, increased to 5mg in month two)
  • Group B: Lifestyle modification only, no medication
  • Group C: Transdermal testosterone replacement therapy

The study ran for 8 weeks.

The Tirzepatide group won across nearly every measurement.

Tirzepatide produced more than double the weight loss of the other two groups: 8.1% compared to 3.0% with TRT and 2.4% with lifestyle alone.

They had the greatest reductions in waist circumference and fat mass, and the largest improvements in binge-eating behavior.

Here’s what happened to their testosterone.

All three groups started at roughly the same level, around 186-192 ng/dL.

After 8 weeks:

  • Tirzepatide: 424 ng/dL
  • TRT: 272 ng/dL
  • Lifestyle only: 266 ng/dL

And the luteinizing hormone numbers are just as interesting.

LH is the signal your brain sends to tell your testes to produce testosterone.

In the Tirzepatide group: LH nearly doubled, rising from 2.6 to 4.5.

The body’s hormonal system came back online on its own.

The TRT group went the opposite direction: LH dropped from 2.3 to 1.8. When you replace testosterone from the outside, the brain stops sending the signal to produce it internally.

Tirzepatide didn’t just raise testosterone.

It restored the system that produces it.

Now the TRT group was using transdermal testosterone. Injectable testosterone has higher bioavailability than transdermal testosterone.

So injections would have worked better.

But high body fat raises the conversion of testosterone to estrogen.
